1. Save the Forms package to your computer Right-click here using right-hand button on your mouse. A pop-up menu will appear. Select "Download link to disk" from the menu then select your "Documents" folder on your hard drive as a convenient location for the files. In a few moments, the forms package will be on your computer's hard drive. 2. Print the Forms package. Open the file you just downloaded. If you saved it to your Documents folder, it will be the file called "Canada Discount Healthcare complete.pdf" inside the Documents folder. You'll need Adobe Acrobat to open the file. It's included on most computers. If you don't have Acrobat, click here to install this free program to your computer. Once the forms package is open, just choose "Print..." from the File menu to print out all the pages using your printer. 3. Fill out the forms: Form #1: is the Authorization/Release and Patient Allergy/Interaction Forms. Our doctor requires this information to issue a Canadian Prescription. This form only has to be completed once a year. Form #2: is the Canadian Medication Order and Re-Order form. Please include a copy of a Valid US Prescription for every RX you wish to order every time you order from us. A few pages require your signature. Note: Any U.S. Citizen resident in the United States may legally import a prescription to the maximum of a 90-day supply for each prescribed drug for personal use only. When you order from Canada Discount Healthcare.com, depending on your doctors orders, you can have your prescription refilled with a maximum of three months supply. This allows us to ship your prescription needs to you over the course of one year. All orders are shipped via the Canada Post - Express Post U.S.A Service and are expected to reach you in 7-14 business days from the date of actual shipment. 4. Fax us the forms. Send the entire set of forms to us using our toll-free fax line at 1-877-536-2004. If you don't have a fax machine, ask your doctor's office if they will fax the forms for you, or use a fax service available at many postal or office services. If you wish to mail your prescription order, this can considerably increase processing time. In a lot of cases, we do not even receive your order for two to three weeks from the time of mailing. This will in turn add two to three weeks onto the estimated processing time. A great option would be to send your envelope by Global Priority Mail with a delivery time of three to five days. The cost for this service is $4.00 and can be arranged by the U.S. Postal Service. This service is convenient for those who choose to send money orders with their orders.
